A claim history/coverage verification report is a historical record that verifies an insured's coverage and claim information. It is used for credentialing purposes and to obtain insurance.

The cost for reports for individual practitioners is $35 per report. Current individual policyholders of Princeton Insurance may receive one Claim History/Coverage Verification per calendar year at no charge.
