How to Request a Loss Run or Claim History/Coverage Verification

1. Download the Authorization to Release Claim History Information form. (PDF)

2. Complete the form and sign it (stamped signatures are not accepted). You must provide a policy number, your current address and phone number, and the name and address of the organization to which you'd like the information released. Please also include the name of the person at the organization to whom you would like the Claim History/Coverage Verification sent.
   
   
3. For current insureds, remember to sign the bottom section of the form if you would like to request your free report.
   
   
4. Mail your request and payment (make checks payable to Princeton Risk Protection, Inc.) to the Credentials Management Unit of Princeton Risk Protection, Inc., P.O. Box 5322, Princeton, NJ 08543-5322.
